I hereby authorize and consent to the review of this employment application electronically. I certify that the information I provided in this on-line application for employment with NTN Buzztime, Inc. ("Buzztime" or "Company") and any documentation referenced or provided (such as a resume) are true and complete to the best of my knowledge. I understand that any falsification, misrepresentation or deliberate omission of facts provides Buzztime with sufficient reason for refusal of employment or cause for termination. I further understand that due to the nature of the Company’s business, I may, if employed, be required to work overtime or shifts, which may include nights, Saturdays or Sundays. Lastly, I understand and agree to the use of an electronic method of signature to demonstrate my acceptance of the terms and conditions of this on-line application.

I understand that if an offer of employment is extended by Buzztime and accepted by me, I will be required to sign the Company’s Ethics Policy, Confidentiality and Work for Hire Agreement and comply with all Company policies as described in the Employee Handbook. I understand and agree that as a further condition of employment, I may be required to submit to a drug screen, and if required for my position, a credit check and/or a motor vehicle record. I understand that a consent form with additional information as it relates to my rights will be provided for my review and signature prior to any drug test or credit check being conducted. I understand that in the event of a confirmed positive drug test, I will be ineligible for employment, and I agree that any conditional offer of employment will be automatically withdrawn. I also acknowledge that my employment is contingent upon satisfactory completion of a background check, as well as providing proof of identity and authorization to work in the US in accordance with federal laws and regulations.

I authorize Buzztime to conduct background checks and to investigate and verify all information provided in this electronic application, my resume or other documentation submitted with this electronic application, and information obtained through interviews and reference checks. Furthermore, my signature below or copy thereof, provides my current and former employers, schools and references to give all information concerning my education, employment, experience and performance (including employment dates, salary, reasons for leaving prior employers, experience and any pertinent information they may have, personal or otherwise) to Buzztime. I also release all parties from any and all liability and waive any claim against all or any parties resulting from furnishing such information to Buzztime or its agents. For background checks conducted by a third party at the request of Buzztime, I understand that a consent form with additional information as it relates to my rights will be provided for my review and signature prior to any investigation being conducted.

Additionally, I understand and agree that if employed, my employment is at-will and therefore may be terminated at any time with or without notice, or with or without cause by the Company or me. Nothing contained in this electronic application or in any policies, procedures or handbooks I might receive, and nothing said to me during interviews, is intended to or shall be deemed to create an employment contract between Buzztime and me. No promises regarding employment were made to me, and I understand that no such promise or guarantee is binding upon the Company unless signed by the CEO of the Company. I understand and agree that if employed by Buzztime, my employment, including compensation and benefits, can be terminated, with or without cause, with or without notice, at any time, at the option of either the Company or me. If employed, I agree to comply with all Company policies and understand that such policies can be modified at any time the Company determines that it is necessary, except for the right of either the Company or me to terminate employment at any time for any reason.
